Medicare Penalty & IRMAA Calculator
Estimate what a late sign-up or a higher income adds to your Medicare premiums. Uses official 2026 Medicare figures.
Part B
10% of the standard Part B premium for each full 12 months you could have had Part B but didn't (and had no employer coverage that qualifies for a Special Enrollment Period).
Part D (drug coverage)
1% of the national base premium for each full month without Part D or other creditable drug coverage (a gap of 63+ days counts).
Part A (if you buy it)
Only applies if you don't qualify for premium-free Part A (most people do). 10% extra, paid for twice the number of years you delayed.
Part B and Part D penalties generally last for as long as you have that coverage, and they rise as premiums rise. Penalties are rounded to the nearest $0.10. You may not owe a penalty if you had qualifying employer coverage, get Extra Help, or qualify for a Special Enrollment Period. Social Security and your plan determine the final amount.
Your income
Medicare looks at your modified adjusted gross income (MAGI) from your tax return 2 years ago. For 2026 premiums, that's your 2024 return (AGI + tax-exempt interest).

Married? Each spouse on Medicare pays the surcharge, so a couple pays it twice.
Income went down because of a life-changing event such as retirement, a lower salary, divorce, or the death of a spouse? You can ask Social Security to use your newer income with form SSA-44. The Part D surcharge applies to any Medicare drug coverage, including Medicare Advantage plans with drug coverage, and is paid to Medicare on top of your plan premium.
